Louis M. "Mike" Ptak

Louis M. “Mike” Ptak 64 of Oglesby. Died 9:30 PM April 4, 2016 in IVCH Emergency Room. Funeral services will 12:00 PM Monday in Ptak Funeral Home. Abbott Philip Davey OSB will officiate. Visitation will be Monday from 10:00 -12:00 PM in the funeral home. Cremation rites will follow the services. Mike was born June 20, 1951 in Peru to James and Norma (Ernenputsch) Ptak. He was a 1969 graduate of St Bede Academy. He married Nancy Schlingman June 24, 1972 at St Valentine Church. Mike worked for Dahlgren’s Company in Oglesby for over 20 years and then worked at Carus Chemical as a shift manager up till his retirement. Mike was an avid White Sox and Packers fan. Mike is survived by his wife Nancy of Oglesby. 1 daughter Kimberly (Shawn) Hill of Bloomington, Il, 2 sons Michael Ptak of Oglesby and Clayton (Brandi) Ptak of St Louis, Mo, 2 granddaughters Natalie and Madison Hill. 3 sisters Diane (Al) Brackett of Washington, Utah, Barb Gorski of Granville and Mary Jo Burris of Peru. 3 brothers Jim (Issie) Ptak of Amboy, Jerome Ptak of Virginia and John (Pam) Ptak of La Salle. He was preceded in death by his parents.
